Looks like a bell trap which is prohibited by most plumbing codes. You might try a crack isolation membrane or an uncoupling membrane between the mortar and tile

1905 in Michigan there’s a chance that the floor drains originally had a bell trap. Hard to tell without seeing it.

Most of the pvc flanges that fit inside a 4” pipe will sit flush if the pipe is trimmed flush with the finish floor

I would not have one without a dedicate discharge and it should be protected with a rpz. The rpz ,the pump, and the installation parts will exceed $500 before labor

If you’re correct and the basement bathroom is vented through the roof, you should be able to connect to that vent assuming it’s in proximity to the new laundry.

The aav would pass in my jurisdiction. Two wax is right there’s a possibility of positive pressure causing an issue but you won’t know for sure until it’s in use. I expect it would work fine.
