Jason, I can't see the end of the handles to be able to identify what you have. It could be one of three faucets. (Gerber, Central or Price Pfistor. The name may be in the center of the handle. Price Pfistor will have a marking of P.P. With out the make I can't tell you what kit you will need.

The end caps on the handles say Central. Do you see any reason not to buy a central kit?

Central has two rebuild kits. Here is a link to a good source. You may also be able to find it at a plumbing supply house. There kit # is RBK1821. This is there number not a part number so if your looking for the correct kit locally I would suggest you bring one of the stems. If you take the diverter stem you won't have to turn the water off. You won't be able to use the tub but you'll have water in the rest of the home. Just keep in mind there are two trim kits for central and only one will work.
