What is it and how do I remove it?

Is a small shower head on the wall that sprays down your body. I can't seem to find out how to remove it. I tried simply using a wrench but it won't budge. I'm concerned I will mess up the pipe behind the wall.

If you're not concerned about the finish try heating it with a torch. Not excessively. Just enough to break that 100yr old bond it has on the nipple it's screwed onto. that hex tells me it's screwed on. the fitting in the wall is usually secured.
