Tub diverter - need help to identify and repair

The diverter handle for my tub broke off. The handle set screw grabs on to a tiny piece o plastic that protrudes from the stem.

1) Can anyone identify the make model from the pictures?
2) Any help on how to fix? Is there any hope of finding a replacement stem?
3) I haven't been able to disassemble any further than what is shown in the pics. My faucet wrench set is too "thick" to fit in the tiny space between the lock-nut and escutcheon. Am I supposed to be able to unscrew the escutcheon first with strap wrench?

tanks: Do you mean the white plastic piece? That is the stem of the diverter. I don't think it would "unscrew". I just moves back and forth to control flow between the tub and shower.

I am going to try unscrewing the escutcheon. But please send more suggestions.
 
Thanks, John. Got it off. Luckily, the broken part is not the main stem, but a stem extender! Delta # RP18627.
