Sewer lateral cleanout

Don't do much sewer line work so excuse my ignorance :O)!

Working in a township that requires a sewer lateral scope. Did the scope and the lateral out to the main is fine, but they called out the (attached photo of the) clean out, saying the slotted lid does not meet code and they needed to install a "DOT BOX" (never heard this terminology - Googling was no help) with a solid type lid.

Any one lend some insight as to what a DOT BOX is and the requirements to install!?

That looks like it is in a driveway or parking spot. If it is, they are saying it needs to be a DOT approved cap; Department Of Transportation.
 
Right? That’s what you would think the meaning of DOTis but It’s located in a path/walkway to the side of the front steps. No vehicle traffic - maybe foot traffic.

Looks like there was a repair, of some sort, made at one point.
 
So, let's assume we need to put on a solid lid... What should be used?

Seriously doubt this will help anyone in the future but, here is the fix...

Original clean out (with slotted lid - not Township approved as it will allow storm water to enter)

You can see there was a previous repair done. No idea if it had Township approval at that time.

Opened up the previous repair and found this...

(not that familiar with sewer lateral terminology but) appears the previous repair included some sort of sleeve. The 4" cast was inserted into a larger 6"(?) outer shaft/tube with a rubber grommet. The visible 4" cast iron was 16" in length and sat on another 4" piece of cast (did not appear to have a Ferco or similar attaching both)

Took a metal cutoff wheel and removed ~2" of cast and inserted a 4" PVC cleanout - which fit perfectly into the outer (grommeted) sleeve.

Finished product - sticks up ~1/2" but there was no (plausible) way to get it lower, giving the 6" insert. (also, not my best cement work but it was pouring rain at the end)

Total repair time - +/-2.5 hours.
