Searching for trim

Pictured below is my hot water stem in my shower. The valve is good. Thera are no leaks or anything. However, the escutcheon and flanges are missing. I have some knobs, but I would like to find new trim. The box store parts do not fit. I have no idea what the manufacturer is of the stem.
Any help is much appreciated.

Here’s the complete rebuild kit for a 3 valve tub/shower.

I’m sure you can just buy the trim from somewhere, I didn’t really look. You could call a few places……Chicago faucet Shoppe.
