Question for the sewer line pro's

Doing a kitchen addition for a rehab and the main waste line is running right through the new crawl space.

Was planning on replacing the cast iron inside but this would also give us the opportunity to (at least partially) tie in and add a cleanout to the line exiting the house. (line exits from rear of house)

Questions...
(.5) that does look like 4" CI, right?
(1) Is there a minimum/maximum distance from the new addition for the cleanout?
(2) OK to use a Fernco 3000-44 to connect PVC to CI, or is there something better?
(3) Using a Charlotte 4x4x4 for the cleanout. Would you reduce to 3" coming up or leave at 4"? (somewhere in the IPC I think I remember this not being allowed!?)

Anything else I'm missing?
