Old Hospital toilet flange replacement

With a requirement of cast iron how are the toilet flanges replaced or repaired in an old hospital. The 4” pipe is within another pipe that the concrete was poured around. There is about a one inch gap between the two pipes. I did one so far using the cast iron compression flange with the rubber sleeve but had to cut and chisel out part of the outer pipe to make clearance. Is there easier way?

cast iron goes OVER/ AROUND the pipe..and is leaded in.

you must chip the concrete away from the pipe
