Need help identifying shower handles/stem

I’m wanting to replace my shower handles; however, I have no idea what to replace them with. Can anyone help me identify the brand/type of handles and stem this is? The stem extends out from the wall approximately 2 3/4 inches.

Also, how do I get the sleeve off? I notice it turns, but I also notice there’s a small screw on the side at the base (it’s so corroded, though, that I can’t get it to unscrew). I’m afraid to damage the system...

I went to my local Home Depot for help, but no one was working in the plumbing department today. :confused:

Any help or advice is greatly appreciated!

Can not be sure but it looks like Price Pfister.
you have to pull a stem to be sure
 
Thanks for the quick response. Once I get the sleeve off, I'll pull the stem. Do I just twist off the sleeve/escutcheon or do I need to remove that little screw in the escutcheon first?
 
Looks like the escutcheon is packed with plumbers putty. Just pull out once the set screw is loosened.
