General venting questions

Hi everyone. I've been doing a bunch of older home restoration for a while now and we've always fernco'd into the existing vent systems that were available. Lately though we're working on fuller guts where the vent pipes have all been removed so I have to rebuild those as well.

Is there a general rule of thumb that a fixture needs to be tied into a committed vent line only after the fixture exceeds a certain distance away from the main stack?

Supplementary vents (v200's, etc) are pretty much unacceptable across the board correct?

Thanks ahead of time for any input. Sorry if this is repeat info but I did try to search the forum but came up empty handed.
 
Every size pipe has a maximum distance from the vent ,called a trap arm,this size varies depending what governing code your jurisdiction uses
