Has anyone ever seen a plumbing vent terminate through the side if a house, rather than the roof, and then turn up like a snorkel? Is this legal? Why or why not?

I have recently come across a couple of radiant heat systems that are being heated using a 50 gallon electric water heater. Is this legal? Is this ok to do according to code? The systems I have seen have been working fine for years, I was just wondering if this as ok to do or not.

Here we have a 3" abs line coming from the second floor of a home, tying into the 4" drain lines for the rest of the house. There is a 4" clean out in the back of the 4" wye. Is this ok? Or does the 3" line need an additional clean out? We go by UPC code here and from my understanding you...
