I am renovating a bathroom in an old cottage and realized I cannot use a standard toilet due to water and drain lines behind the toilet. I have about 6" rough-in so looking for toilet options or flange with 4" offset. The floor is the exposed ceiling of the kitchen below so hoping to not move plumbing if possible. Any options or other solutions?

That I know of your not going to find a 6" rough in toilet. Their 10",12" & 14". If your old toilet fits then measure off the back
wall to get your center. Are you located in the States.? Can you move your water lines over if their in the way.

Your picture shows that your sink currently has an illegal S-trap.

What is the distance from the wall to the centerline of the toilet flange?

What is the routing of the toilet drain? It appears that the existing sink drains into it. How far does it run horizontally?

The only answer I can see to avert future issues is to relocate the water lines and modify the drains.
