I have these (see pics attached) widespread 1/4 turn faucets throughout my house, built in 1986. The tub drain lever escutcheons have Kohler on them.

One is leaking and two no longer have stops at shut-off.

Can anyone identify the brand/model?

What repair kits do I need?

How do you get out the cartridge? It didn't want to come out from the top.

Okay here's the link to parts list

Look down inside around cartridge. Old style use a retainer RING THAT REQUIRES A TOOL TO REMOVE. opps CAPLOCK

whit that removed you just pull up the cartridge. Put the flange back on to hold the valve to the sink
Good luck

Never mind the3 tool. You already removed the retainer nut.

get a pair of needle nose and pull out stem. try a screw in the stem. might get a better grip on it
