Slow running hot water

I have a rather strange thing happening with my hot water supply. Sometimes a hot faucet will run very slow when turned on. This happens maybe 5%-10% of the time a faucet is used. It will run like that for a couple minutes if I leave it ( or possibly forever since I've never tried to leave it longer). This has happened to most of the hot water sources (probably all of them since the only ones not affected yet are rarely used). In my bathroom where I have two sinks, if this happens to one faucet, I'll turn the other one on and it will run fine. So one is running full and one slow at the same time. And every time this happens, if I turn the slow running faucet off and back on, it immediately runs fine. My hot water heater is 20 years old but I can't see how that is a factor given the symptoms. I have a PEX system with a manifold distributing each water source.

Any ideas?
