Question about drainage cleanouts

Here we have a 3" abs line coming from the second floor of a home, tying into the 4" drain lines for the rest of the house. There is a 4" clean out in the back of the 4" wye. Is this ok? Or does the 3" line need an additional clean out? We go by UPC code here and from my understanding you don't need a clean out if the horizontal line is less than 5'. I guess I'm not exactly sure about this 5' rule. Can someone clear this up for me? The 3" horizontal line in the picture is about 2' and the pic is in the crawl space.
 
Reason being is that after your offset being more than 5 ft it's considered nominally horizontal. I'm going to say you don't need a clean out, just the 4" one thats there, but someone else may post with a different opinion. However I don't think it would be a bad practice to put one in. Chances that you would have to use it would be pretty slim but ya never know.
 
Thanks Steve. I'm more concerned with passing my inspection and being up to code rather than actually needing to use the clean out. As far as I'm concerned its good enough, I was just looking for a second opinion.
 
You're good to go, no additional clean out is needed. What I don't like is the 3" fitting hub glued into the 4" pipe. You should have used a 4x3 bushing in the Y.

Thanks John. There actually is a 3" x 4" bushing in the 4" wye, with a short piece of 3", a 3" coupling, and a 3" street 45. I was running low on fittings at the time.
