Venting Capecod bathroom

Hello, I'm curious as to the best way to vent my capecod bathroom? The main stack is 14 ft away from the toilet and was wondering if I could place a sanitary t on the opposite side of the header in order to not interfere with where the door will be? Also where should I place cleanouts for the shower and do I need one for the toilet? Toilet will be dead ahead of the door vanity to the left and shower to the right. Thanks for the help.

I can not believe you drilled that beam
not only did you drill it, you drilled it at the top and not the center..

you should have ran the pipe the other direction

is this work being inspected by the building inspector and plumbing inspector?
 
Running the pipe the other direction wasn't an option unfortunately due to location of the stack and chimney. No inspection, just trying to be functional.
 
The sweep with nothing connected to it is for the vent, both 2 in lines are still there
