New drain cleanout

I am plumbing a new bath and adding a bi-directional cleanout at the same time. This is in a crawlspace with a 60" ceiling.

My question is, does it matter which direction the cleanout wyes point? Should they be 90 degrees to the ground, or point down?

Cut that out and install this in the upright position. What you have will fill with sewage and is improper.

The only reason I was hesitant to orient it up is that there is very limited space above it.

That is why the picture/fitting shown will do the same thing for less money the fitting you show is for buried piping

If there’s no room then move the clean out to where you have more room. Turning the fitting down is never an option.
