It’s a very low end delta old 1980-1990 delex here is amazon link to purchase new. Lots of Vaseline to get those ears in. I always advise to remove the big rubber end and install a diverter spout. But here ya go

Never seen one but I was going to guess it was a diverter for a Delta with an extension
