I have several holes in the ground that are access points for valves in a water supply system. I need a sturdy liner and lid for the hole to keep dirt from filling back in on the hole. Also some sort of insulation for the area as it gets quite cold in the winter here. The holes are about 3' deep and I would like about a 30" diameter around the valves.

Any suggestions for solutions and manufacturers are most appreciated.
 
What I normally see is a piece of 6" sewer pipe stood up on top of the valve, and then a valve "box" made out of ductile iron with an iron cap placed on top of the piece of sewer pipe. Asphalt or concrete poured around the iron riser box will support it even with vehicles driving over top of it. The top of the box is ~7 or 8 inches in diameter, and you can reach down into it with long valve key to operate the valve.

However, it sounds as though you are looking for something much bigger. I would bet Chris will know of something.
