Uneven Toilet Flange and Sewer Pipe

My plumber just finished installing a new sewer line to a toilet addition. The 4" sewer line and flange are coming up uneven with 1/4" slope. The lowest point falls flush with the concrete. The plumber says he can just tapcon the flange to even it out, but that will still leave the sewer pipe unlevel. Is this going to cause me issues when going to install the toilet?

Is that just dirt that the flange is sitting on? If so when the floor is pored the flange will be to low.

John, I believe that is fresh concrete the flange is resting on/in.

I'd redo that flange. That will be a constant problem the way it is set.
