A guy asked me to help him get this piece off so he can attach a hose.
He said he tried a wrench and pliers. Looks to me like a piece of an old hose stuck on the spigot ( my assumption).

Any ideas on a good way to get this off and if my assumption is right?

Do I need to replace the whole spigot?

A garden hose will screw over those male hose threads just like any other hose faucet.
 
It may have a set screw that you can get out or, more likely, one of the "frangible" kind that, when you tighten it, it breaks off preventing the casual user from removing it. You may have to "Dremel" it off.
 
I'll try screwing a hose onto it and ask why he wants to replace it.
 
I did not know what these were and Googled it. Good video where he recommends replacing the breakaway screw with a set screw.
 
When I replaced my vacuum breakers, I initially opted to replace the frangible screw with a 1/4-20 set screw and then simply decided not to put any sort of a retaining screw back in at all. I am sure somewhere a low level government regulator is doing a slow barbecue roll in his or her grave because of that but they will survive!
 
The only time I install the breakaway screw is when it’s required.

The only time I use those vac breakers are when it’s required.
