How do I remove handle from this unit?

Can’t figure out how to get this handle off. No set screws and no brand name. 25+ years old. Pulling on it hasn’t been successful.

I can, later today, but their are no screws in the top. The handle is a solid piece, with no set screws.
 
You may have to hold the bottom part of the handle while turning the handle lever counterclockwise.
 
Next question, recognize this valve stem? Not critical because I replaced entire faucet, but curious as to brand. Lasted 25+ years before it started to leak!

No way to tell looking at the stem from that position. Take some more pics of the entire faucet including spout.
