Exterior Tap - How do I find Parts

I have an exterior tap that is freeze-proof that is leaking and require parts to fix it. I can't seem to find a out the manufactuer or model number. Anyone recognize it?

Post good CLEAR pics of it from both sides.

If you left that hose on during a freeze, the faucet might be history.
Look at that lettering on the black ring, they tried to warn you.

I don’t recognize that style with a nut on top, that is usually an anti-siphon vacuum breaker in that location.

Sorry don't have better pictures it is at My mom's house. I think I need to replace the seals at the valve inside, but want to make sure I get the correct parts.

Looks to me like “Remove hose to self drain”.
