Do I really need .25 inch slope for drain?

Again 1/8” will work just fine. Why use 3” when there is absolutely no need to and that doesn’t change the fall at all. You will have absolutely no problem with 2” and 1/8 of fall. Everyone does it if it is needed. Your washer had a pump that will be pumping water out also. 2” pvc can handle...

    Low Toilet Bowl Levels

When someone says gray water it just means dirty water. So for instance your washer is draining gray water because it is only dirty water no solids or anything. So when gray water is draining it will go by a partial stoppage because there is nothing in the water to slow it down or cause the clog...

    Low Toilet Bowl Levels

I do not see a vent causing this. It has to be a partially stopped sewer I would think. You don’t notice it anywhere else because everything else is gray water and goes by the partial stoppage.

    Replace Disposal or Not

If it runs fine, looks ok and doesn’t leak and you are fine with it I would keep it. Make sure it does t smell or anything also and I believe someone else said it but just get a new disposal Flange so it all looks new and go with it. Disposals are easy to change if u need to change it out anyway.

    Getting Frustrated

Sounds like you probably have roots growing in your sewer main. Happens all the time here and they can be a pain. You can run cable through it and unstop it and it may last a year or only a couple days. I have always ran a big cutter head whenever I was hitting or thought I was hitting roots.

How do I install a flush mount cleanout cap?

It is definitely possible but now it is going to be more work. The clean out you have a link for as u can see is adjustable and should have been put in before the concrete was poured. Now the easiest way for you will be to find a clean out adapter that will slide inside the 4” pipe and use an...
