That appears to me to be a Moen. As far as I know, the model number doesn't matter, since they all use the same cartridge. The only difference is whether it is a Posi-Temp valve (you rotate the handle to turn the water on and off) or a Moen-Trol valve (you have to pull or push the handle to turn the water on or off).

I am not sure what your issue is, but Moen has a lifetime guarantee. If you need a new cartridge or other parts, call their tech support people, and they will mail you what you need. Usually at my door 3 or 4 days later.
 
Thanks for the replies! Briefly - this was in the wall when we bought the house - i think it was supposed to be a small shower type thing for washing a dog. The room its in is off the garage and was unfinished. I have just gotten around to finishing it and realized the parts i thought were for this hardware actually are not. But it helps you think this is Moen because the parts i have are Grohe and nothing in the instructions looks like whats in the wall. So that helps ;)

There’s two different cartridges for their showers. 1222B for the posi-temp and 1225B for the older shower controls and most basins.

As already mentioned, that is a Moen PosiTemp valve. Any PosiTemp trim you find will work on it.

Were I you, I would be curious as to what that odd buildup on the cold connection is.
