Clawfoot tub shower mixer/riser issue

I have an issue with a claw foot tub in a house that was recently purchased. The shower riser kept popping out of the mixer. The original owner mounted the mixer on the slopped end of the tub so the riser comes out at an angle and does not line up correctly with the shower curtain assembly. So basically the shower riser comes out of the mixer angled towards the wall, if it is exiting the mixer properly.

Are the any suggestion to correct this alignment issue?

Also is the riser supposed to insert into the top of the mixer, this one sits flush then there is a piece that threads on the mixer then a rubber washer and nut thread on to it. The piece is threaded on there but you cannot see the nut or washer in the pic they are not on there.

we have similar tub valves on our campus when you said threaded is it a 3/8 thread????? that's what our are.... another way if its 3/8 ips chrome is 3/8 chrome ips st 90 a nipple to another 90 to square it off to the wall
 
It's likely not a tapered pipe thread.
In fact when I zoom in on it, it appears to be some sort of compression fitting with the follower nut missing.
 
yes the follower nut and rubber bushing are not on in the pic. So that riser pipe does not insert into the mixer at all and just depends on the compression from the follower nut and bushing?
