Cause? Condensation?

It’s a horizontal vent for high efficiency (condensing) furnaces or water heaters. My guess is it’s Installed incorrectly with either intake and exhaust reversed or improper allowance for drainage of condensation.
 
Reversal of the intake and exhaust makes perfect sense. That is what I would be focusing on. Verify before doing anything.
 
I'd potentially agree but the unit was installed 3-years ago and this just started last month.

Just a guess here but my condensing furnace (in Michigan) didn’t make visible condensation when it was summer…you know like 3 months ago? But in November/December when cold and dry out it sure as heck did. Again just a guess…where is this, SE Pennsylvania?
