There are already flood resistant toilets on the market (Latest is the Delta FlushIQ) and non are completely flood proof and have their own issues relating to the designs and technology. If you could invent a 100% flood proof toilet that has all pros and no cons, then you will be a billionaire and Kohler, Delta, Toto, Am. Standard and the like will be beating down your door for the patent.
 
Yes, I have seen this happen on more than one occasion. The best way to avoid this is to keep the innards of the toilet tank in good repair, and fix any piping issue that may cause repeated clogging.
 
No, a slowly leaky flapper has its own way of presenting itself. Most clogged toilet floods happen when somebody does a "flush and run", like Mr. David described and they don't realize the flapper is hung up and the toilet is clogged until the water comes flooding out. This also indicates that the person just took a dump and didn't thoroughly wash their hands, so don't accept any invitations to dinner anytime soon.
 
Do you wait long enough to notice a slow leaky flapper?

No. If it flushed, then a slow leaking flapper is not going to back up the toilet. Just waste water. a slow leaking fill valve will do the same thing.
It's takes 2 things to cause a flood from a toilet bowl . A stoppage and water flow.
 
You can always go for the old no flush style toilets.
His and hers. No Flush = No Flood.
