What's going on with my drain field?

My septic tank drains to a dosing tank, where a pump pumps it up to a distribution box, that is at grade level, then into the drainfield. Cutting the grass for the 1st time, I found a hole, about 12" diameter in the drainfield. It appears to be approximately near the end of the first run/trench. According to the drawing I have, the trenches are daisychained together. There is no standing water in the field, but it does look like water is being pushed up around the distribution box. It wet and the grass grows more there.
It has not backed up into the house. Any Idea's of what could be going on.
