I replace the valve stems for the hot and cold water a month ago. They worked perfectly until recently. The cold water works fine but the hot water stem seems to have a problem.

As the knob turns it doesnt come on at the point it used to. If I turn it a bit more it comes on and starts pulsating. You can feel the pipe in the wall vibrating. If I turn it on a bit more it comes on full with no pulsing.

What could cause this? Is the valve stem failing?

It's possible you have a loose washer or faulty valve stem. I would try replacing it to see if it corrects your problem.
 
I just experienced the same thing at my Havasu house. My new faucet would not shut off completely. I opened it up and the screw holding the washer had unscrewed. I tore everything apart and found both the washer and screw in my anti-siphon valve!
