I'm trying to replace my shower diverter for my 3-valve shower but I can't find anything that looks like the valve I have in store or online. Does anyone recognize this shower diverter? The only brand name that I can find is CareFree.

best thing to do is 1st check for a name brand usually
on the handles but check the valve itself bring that part to a
plumbing supply house they are more knowledgeable
than big box stores they usually have books at the counter
take a pic of the valve also that way they get the whole pictue
 
Sorry, but be prepared to plan on replacing the valve.
That is going to be virtually impossible to find, it is a rarity.
Delta and Moen make a repair plate that covers widespread faucet holes.
If you had used deep faucet sockets to remove the valve assemblies you may have had a fighting chance.

I agree with breplum that it looks like that stem is shot. The only chance you got on that is if you have a real good hardware store in your area. Take the whole stem with you and see if by any chance they have one. If so make sure you use tub wrenches to install so you do chew it up.
