Help identify faucet / stem

Hi:

I have faucet set (see pic link belowon a whirlpool tub. The tub is used very infrequently and the hot water valve has started to leak from the stem, but only when turned on. I'm assuming this means that the seat is good, but one or more of the o-rings is worn.

1) Could someone identify the make/model of this faucet & stem?
2) Does anyone know the part number of the rebuild kit (o-rings, spring, seat)?

That is Delta
Model# RP25513-3 Replace the stem spring and seat.
