Default Manufacturer of this?

All you experts, old time plumbers, etc: From this picture, can any of you tell me who made this valve and how to disassemble it? It has no screws under the plastic cap to remove the handle assembly from the valve. Without removing the handle, the rectangular plate and chrome cylinder around the stem can't be removed.

It's American Standard, here is a breakdown of the parts. There is a allen screw under the handle to remove it. Hope this helps.
