A picture would help with proper diagnosis, but it sounds as though you may need to replace the whole flush valve assembly.
To do this, shut off the water and unhook the supply line from the tank. Unbolt the tank from the bowl, and lift the tank off. Take off the foam rubber gasket that seals the tank to the bowl. Underneath that you should find a really big, usually plastic, nut. Use a big pair of pliers to take that nut off, and the old flush valve will come right out. Install the new flush valve by reversing the above, with a new foam rubber gasket.
